Coumadin should not be given along with hypoglycemic agents (chlorpropamide and tolbutamide) and anticonvulsants (phenytoin and phenobarbital). It is contraindicated in patients with increased risk of bleeding, as it is a narrow therapeutic index drug. This is not administered in patients with real disease, cerebrovascular disease and gastrointestinal bleeding. The patient should be advised to regularly check prothrombin time. Coumadin should not be given to pregnant women.\u00a0 Avoid alcohol and any activity that may result in traumatic injury. The patient should be advised to take a normal balanced diet containing vitamin K. The patient should avoid intake of canberry juice or any other canberry products. This is not recommended in pediatric patients.
